B. Applicant has previously attended a MIAM

I confirm:

• the applicant only attended a MIAM; _____

• the applicant and the respondent(s) together attended the MIAM; ____

• the applicant and the respondent(s) have each attended separate MIAMs; or _____ _____ • the respondent(s) has(have) made or is(are) making arrangements to attend a separate MIAM.

And I also confirm that mediation or other form of non-court dispute resolution is not proceeding because:

• the applicant has attended a MIAM alone and:

* (s)he does not wish to start or continue mediation; or ___ * I have determined that mediation is unsuitable as a means of resolving the dispute. _____
